AFFIDAVIT OF ACCURACY AND RELEASE
The information I have given in this application is true and complete. I understand that any inaccurate or misleading statements on this application or other pre-employment materials may lead to a refusal to hire or, if hired, dismissal at anytime. I understand and agree that, if employed, I will follow all rules, regulations and policies of the company and that these may be changed at any time.
I authorize investigation of all statements contained herein, the references listed and all information concerning my previous employment. I hereby release all parties from all liability for any damage that may result from furnishing any information, personal or otherwise, to you.
I EXPRESSLY AGREE AND UNDERSTAND THAT, IF EMPLOYED, MY EMPLOYMENT, HAVING NO SPECIFIED TERM, IS BASED UPON MUTUAL CONSENT AND MAY BE TERMINATED AT WILL, WITH OR WITHOUT CAUSE, BY EITHER PARTY (THE COMPANY OR ME) WITHOUT PRIOR NOTICE TO THE OTHER. I ALSO UNDERSTAND THAT THIS ASPECT OF MY EMPLOYMENT MAY NOT CHANGE ABSENT AN INDIVIDUAL WRITTEN AGREEMENT SIGNED BY BOTH ME AND THE PRESIDENT OF THE COMPANY. THIS APPLICATION DOES NOT CONSTITUTE AN AGREEMENT OR CONTRACT FOR EMPLOYMENT FOR ANY SPECIFIED PERIOD OR DEFINITE DURATION.
In effect all applicants must be given a written offer for employment. At no time is a verbal conversation a contract of employment.
I further understand and agree that, if hired, upon termination of my employment, I will promptly return all property in my custody belonging to the employer, including, but not limited to, office keys, key cards, manuals and computer equipment.
This application is current for 60 days. At the conclusion of that time, if I have not heard from the Company and still wish to be considered for employment, it will be necessary to complete a new application.
All applicants will be required to undergo a drug screening test. A separate policy in that regard will be provided.
